Abstract
A review of the literature concerning pretibial myxedema is reported. This pathology of pluridisciplinar interest, occurs usually in association with thyroid functionality disorders, especially Graves’ disease.
The etiopathogenesis is still obscure: the accumulation of glicosaminoglicans in pretibial skin appears to be a responso by local fibroblasts to a stimulating factor in the patient’s serum, identificable in somatomedin.
The lesions occur most frequently on the anterior tibia and dorsa of the feet, sud are morphologically varied, including plaque-like. nodular, diffuse or elephantiasic forms. Diagnosis is based upon clinical features, association with thyroid pathology, and histologic pattern of excessive amounts of mucin into dermis.
New data support the therapeutic uso of octreotide and suggest that it may suppress lìbroblast glicosaminogiicans secretion within the skin via depletion of insuline-like growth factor or the blocking of its effects.
