Abstract
Smoking annually kills almost 6 million people, generating very high social costs. Of these, 5 million are considered smokers and more than 600,000 nonsmokers. About 80% of the tobacco addicts start smoking before the age of 18, developing both physical and psychological dependence very quickly. Tobacco smoke contains more than 4,000 chemicals, of which 250 are harmful and 50 carcinogenic. Nicotine has all drug-equivalent pharmacological characteristics and both ICD-10 and DSM-5 recognize tobacco dependence as a disease. Many smokers are unaware of the brutal consequences that smoking brings to one's body; some believe they are immune to it, others think it's too late to quit and others still want to give it up but they do not know how to ask who to ask for help. Pharmacologic management is based on first-line treatment in the form of nicotine replacement therapy, bupropion and varenicline and second-line treatment as clonidine and nortriptyline. Every health professional has obligation to help their patients to quit and the intervention should be diagnostic and therapeutic. The best results are obtained by behavioural and social support combined with pharmacotherapy whenever needed.
